Most problems cross a boundary.
The four practices exist separately because buyers shop for them separately, but the interesting problems rarely respect that split. A finding from a penetration test is usually an architecture decision that looked reasonable at the time. A pipeline that leaks credentials is usually an identity design problem wearing a CI costume. A team that can't keep its platform secure is often a hiring problem that was diagnosed as a tooling problem.
Because the same people work across all four, a problem doesn't get handed off at the boundary — which, in our experience, is exactly where it tends to live.
